Sports Medicals & Required Forms

PLEASE READ CAREFULLY: 

 Pre-Participation Examinations
  • Before you are allowed to participate in any team activities [e.g., practices, competitions, conditioning,weight training], you must be medically cleared and have the appropriate medical and insurance information paperwork on file in the sports medicine office.
  • Emmaus Athletics requires all student-athletes on our intercollegiate sports team to obtain physical examination clearance from an appropriately qualified medical physician prior to reporting for their respective season.  

Health Insurance
  • All student-athletes must have personal health insurance. Proof of insurance is also required for all residential students at Emmaus, and is mandatory for all student-athletes prior to participation in any team activity.
  • If you do not have a current health insurance coverage plan, you must contact the Residence Life Department to inquire about plan options with Emmaus Bible College.  Email the Residence Life office at: reslife@emmaus.edu 
 
Required Forms For All Athletes

 Complete the following 3 online forms:
 

1) Athletics Participant Agreement, Waiver and General Release Form

2) Consent to Treat Form

3) Concussion and Injury Reporting Acknowledgment

 
Print or scan and submit these forms prior to athletic participation. The Physical Exam Form must be signed by a physican. 
 
 
Select one of the following options for submission: 
 

1) Scan and email completed forms to Krista Iverson at: kiverson@emmaus.edu     

OR

2) Bring both forms to campus.  Submit these forms to athletic trainer Krista Iverson upon your arrival.
